Owner requests vet referral
Our goal is to work with you and your veterinarian so that your animal can achieve the very best care. Should pet owners want to move forward with us, we ask for the to start by requesting that their vet sign off their pet's care.

Vet signs off
Similar to an MD prescribing physical therapy, your vet will complete the vet form request in order for your pet to get started with us. When a vet form request has been sent, your vet will receive an email requesting a signature + a digital copy of an animal's records.

When we have your vet form completed, our team will reach out to you to get your animal's appointment scheduled! Thank you in advance for completing the additional history and consent paperwork before the time of your appointment.
